17th Sláintecare Right Care Right Place Right Time webinar

In partnership with Age Friendly Ireland, we will explore Healthy Age Friendly Homes, a multi-agency programme which works with older people, improving their living conditions through access to continued health and social care supports, and tailored community supports.

We will hear from people who have benefitted from the programme, as well as colleagues from a range of organisations involved in delivering this innovative person-centred programme.
